Princeton Medical Center Foundation: Princeton Health Innovations 

The Princeton Medical Center Foundation FY24 grant cycle is now closed. CONGRATULATIONS to this years awardees! 

The Princeton Health Innovations program provides seed funding to initiate new projects or implement enhancements to an existing program or project at Princeton Health that demonstrate innovation or creative problem solving.  

 


 

The FY24 grantees and projects are:

(Department of Medicine) - Preventing Hospital Admissions After Heart Failure, etc.

  • Purchase artificial intelligence specifically designed to identify patients with complex health conditions who are likely to be readmitted for heart failure or sepsis (infection), the two most common reason why patients are admitted to Princeton Medical Center. This tool will improve patient outcomes by using patients’ health data to recommend next steps based on evidence-based guidelines while following Penn Medicine protocol.

(Acute Rehabilitation Unit)Swallowing Again: Improving Dysphagia outcomes of Stroke Specialty Program at Acute Rehabilitation Unit 

  • Purchase equipment and other innovative devices to enhance dysphagia therapy. Accessing these new evidence-based treatments and therapies which aid in muscle strengthening to rehabilitate swallowing provide advances in dysphagia intervention.

(Princeton House Behavioral Health) - Real Time Wellness Checks for our First Responders Program

  • Purchase GoMo Health Chat Bot which serves as a personal concierge by regularly engaging patients via text after they have completed the First Responders program. This platform offers users real-time support services and will alert to our staff if outreach or intervention is needed.
