The following tips can help you prepare for your outpatient evaluation appointment.

For adults:

Please bring two forms of identification, your insurance card, your driver’s license, and the names, addresses, and phone numbers of your medical doctor, pharmacy, therapist, psychiatrist, and any other providers we may need to coordinate with regarding your care, such as a probation officer. In addition, bring a list of all medications and dosages.

This evaluation will take approximately 2 1/2 hours. You should plan to start our group program the day after your evaluation for three to five days per week based on the doctor’s recommendation.

For children and adolescents:

Please bring two forms of identification for the parent/legal guardian who is bringing your child. You will also need your child’s insurance card and the names, addresses, and phone numbers of your child’s pediatrician, pharmacy, therapists, psychiatrists, school, guidance counselor, and any other providers we may need to coordinate with regarding your care, such as a probation officer. In addition, bring a list of all your child’s medications and dosages.
This evaluation will take approximately 3 1/2 hours. You should plan for your child to start our group program the day after the evaluation for three to five days per week based on the doctor’s recommendation.

If you need to cancel or reschedule:

Call our Admissions team at 888.437.1610 if you need to cancel or reschedule an evaluation appointment. We require 24 hours notice for cancellations or requests for rescheduling. Without this notice, a new evaluation time may not be immediately available.
