How to Obtain Your Medical Record

Patient Access to Health Information

Penn Medicine Princeton Health understands how important it is for patients to have access to their health information and to be able to share it with others.

We offer multiple options to access and share your health information, including via the patient portal or an authorization form. Learn more about each option below.

 

Patient Portal

In the portal, you can readily and securely access much of your health information at your convenience and for free. You can also download a PDF copy of your health information for yourself or for you to provide to other health care providers.

Penn Medicine Princeton Health and Medical Center patients can log in or sign up for an account. You will then get immediate access to your records.

The patient portal also offers a Share Everywhere feature that lets you give providers one-time, limited access to your health information that is in the portal, such as your medications, allergies, immunizations, and other health issues.

 

Health Information Management

If you want your records or radiology images to be sent to another health care provider or someone other than yourself, you must submit the authorization form on the link below.

To request a copy of your medical records, click and print the Authorization form using this link ROI authorization form.  Fill out completely.  And be sure to sign and date with a hand-written signature. 

Send your completed authorization form via fax 609.853.7051; or mail to: HIM Department, Princeton Medical Center, One Plainsboro Road, Plainsboro, NJ 08536, or email to recordrequest@pennmedicine.upenn.edu. (We do not recommend emailing as there is risk emailing personal information.)

The processing time for copies of records is 5-7 business days, depending on the type of records and the dates of service requested. Federal law permits Penn Medicine - Princeton Medical Center to assess patients a reasonable, cost-based fee for copies of their records (see 45 CFR § 164.524(c)(4)).

MRO is the company that handles release of medical records for Penn Medicine - Princeton Medical Center. As our partner for Release of Information (ROI), it is our pleasure to serve you!

Patient requests totaling 10 or more pages will be assessed a cost-based fee of $6.50, plus shipping charges if mailed by MRO’s National Service Center in Valley Forge, PA.

Once the records are ready, you will be notified via e-mail or mail.

Please note, records from another facility contained within the requested records may be released.
